Performance Indicator for Resilient Response(訓練評価指標開発)

In this study, data collected during hospital disaster exercises are analyzed to elucidate the adaptive responses and potential problems of the emergency medical care system. The analysis compared what was done during an exercise and what was imagined before the exercise as the expected normative response. To understand what the exercise participants actually did, we analyzed the observation and communication data collected at a disaster base hospital in Japan during the response to a mass casualty incident. 

災害訓練の目的は、初動や緊急時の対応に関する行動・意思決定を模擬的に体験することによって、定められた対応手順を理解・身に付けると共に、災害対応やBCPに関する不備や問題点を発見することである。一方、大規模訓練の活動の全体像を把握することは容易ではなく、訓練企画者による観察や参加者による振返りだけでは問題の発見や訓練評価を十分に行うことは難しい。訓練を通して災害対応やBCPの適切な改善につなげるためには、訓練時の活動をできる限り記録し、様々な視点から分析することによって、訓練で何が起きていたか(what was happening)、何が行われたか(what was done)を詳細に把握し、データに基づく現状の把握、問題発見、訓練評価を行うことが必要である。本研究では複数の災害拠点病院の多数傷病者受け入れ訓練を対象にデータ収集、詳細分析を行うことでデータに基づくパフォーマンス指標の開発を行っている。

Modeling and Simulation of Hospital Response to MCI (多数傷病者受入れシミュレーション)

We developed a simulation model of detailed in-hospital disaster response to a mass casualty incident (MCI). This simulation model includes detailed models of patients, floor configurations, resources, response tasks, and inter-department communication, which consider resource requirements for the treatment of different patients with various injuries and physical conditions. We conducted simulations of the target hospital to test various resource allocation strategies and found that the appropriate resource allocations and quick replenishment could alleviate the negative effect of resource shortages and maintain a higher performance. 

災害対応(多数傷病者受入れ)プロセスを忠実に再現できる計算機シミュレーションは、訓練評価だけでなく訓練シナリオ設計やBCP評価など様々な活用が期待できる。現在、多数傷病者受入れの詳細なタスク分析に基づき、訓練記録(descriptive simulation)と模範対応(normative simulation)の両方を再現可能な計算機シミュレーションを開発している。このシミュレーションは、「1次・2次トリアージ」「患者移送」「診療」「検査」「医療資器材調達」「入院・手術調整」「転帰」とこれらに関わる救護所指揮所および災対本部との「コミュニケーション・意思決定」の過程を対象としている。各過程は必要なリソース(人・モノ・部屋の容量等)があれば開始され、なければ利用可能になるまで開始されない。
